The US military said it struck 90 targets along the Iranian coast. Explosions were reported in several Iranian ports, including Bushehr, Bandar Abbas, and Sirik.
Iran's Islamic Revolutionary Guard Corps said in response they attacked US military bases in Kuwait and Bahrain, according to a statement published by state broadcaster IRIB.
The IRGC's naval and air forces said they conducted a joint attack with drones and missiles on US infrastructure and facilities, including Camp Arifjan and Ali Al Salem Air Base in Kuwait, as well as Sheikh Isa Air Base and Juffair in Bahrain.
Juffair hosts the US Naval Support Activity, the main US Navy base in the Persian Gulf, where the US Fifth Fleet is stationed.
The IRGC said the strikes were in response to US attacks on Iran's southern coastal provinces and two bridges in eastern Iran, according to IRIB.
Earlier, the IRGC said the US struck a railway bridge in Akkala, an area northeast of Tehran, reports the semi-official Tasnim news agency. Video from Iranian media, geolocated by CNN, shows a damaged railway bridge near Akkala.
In a statement, CENTCOM said US forces struck "90 Iranian military targets" along the coast. The bridge was not mentioned.
According to authorities, sirens warning of an approaching threat sounded in Bahrain and Kuwait on Thursday morning. The Kuwaiti army said its air defense systems were responding to "hostile missile and drone threats."
The IRGC warned it would expand its response to other US bases in the region if the US launches a retaliatory strike, according to a statement published by IRIB.
The US military said it has struck at least 170 targets in Iran over the past two days.
US President Donald Trump warned Iran that strikes would "significantly intensify" if it attacks more vessels in the Strait of Hormuz, after stating he considers the ceasefire "over." Iran's chief negotiator said Iran will respond to any attacks and that the strait "will only open under 'Iranian arrangements,' not under threats from the US."
Meanwhile, a multi-hour farewell ceremony for the slain Supreme Leader Ali Khamenei will conclude today, and all attention is focused on whether his son and successor Mojtaba will appear at the funeral.
