Toledo Deputy Police Chief Joe Heffernan said no suspects were in custody several hours later, and authorities urged anyone at the festival to provide any photos or videos from their phones for possible leads.
The shooting took place near the Old West End Festival, an annual event featuring live music and home tours.
Heffernan said it appeared at least two people fired weapons and were "likely shooting at each other."
Two of the victims are in critical condition, Heffernan added. The ages of the victims ranged from 14 to 61, with most in their 20s.
"I am deeply concerned about the situation in Toledo tonight," Ohio Governor Mike DeWine said in a statement. "Summer festivals should be safe places where families can spend time together without fear of violence."
Numerous videos appeared on social media showing people running to the sound of gunfire and emergency responders treating the injured.
Fire Chief Allison Armstrong said getting to the hospital was difficult due to road closures and traffic jams caused by people leaving the festival, but rescuers managed to evacuate all victims from the scene within an hour.
Kevin Berry was sitting in an arboretum in his neighborhood listening to live music with friends when he heard several gunshots.
"Everyone hit the ground," he said.
When Berry looked up, he saw a handgun thrown to the ground less than 15 meters away. Police officers already at the festival site responded immediately.
Berry, who has medical training and served in the Navy, walked around the area looking for anyone who might need help and saw at least five people with gunshot wounds.
Addendum
The Old West End Festival is a two-day event in Toledo's historic district that includes live music, food vendors, home tours, and shopping. Berry called it the "kickoff to Toledo's summer festival season."
George Kral, the city's safety director, said authorities are discussing with organizers whether the festival will continue through the weekend.
"This is one of the most iconic festivals in Toledo," he said, "and it's a shame that an event like this has marred it."
