Starting from 2025, public healthcare institutions may provide paid services: what is changing

Starting from January 1, 2025, state and municipal medical institutions will be able to provide paid medical services in Ukraine. The fees will apply to services that are not covered by the medical guarantee program and additional services.
Starting from January 1, 2025, state-owned and municipally owned medical institutions in Ukraine will be able to provide paid medical and additional services to patients not covered by the medical guarantees program. UNN explains when medical institutions can provide services for payment.

According to the Resolution of the Cabinet of Ministers No. 781, which was adopted in July 2024 and entered into force on January 1, state or municipal healthcare institutions may conduct additional (economic) activities provided for in their charters and independently set fees for them.
The list of health care services that can be provided for a fee or free of charge at the expense of the state or local budget, in particular within the framework of the program of state guarantees of health care for the population, is published on the website and on information stands inside health care institutions.
"Provision of healthcare services to the public for a fee from legal entities and
of individuals shall be executed with the relevant supporting documents, entered into the electronic healthcare system indicating the source of payment, and payment shall be made exclusively in non-cash form," the resolution says.
Cases in which full payment for health care services will be made:
- without a doctor's referral, unless a referral is not required by law;
- under contracts with legal entities;
- provision of health care services to the population in health care facilities managed by the State Administration of Affairs and state bodies belonging to the security and defense sector to patients who do not belong to the assigned contingent;
- that are not covered by the state healthcare guarantee program.
Cases in which patients will pay only for additional services, while the cost of the medical service itself will be covered by the medical guarantee program. We are talking about:
- Provision of medical services at the patient's place of residence (home) or stay, provided that this is the patient's wish and not due to his or her medical condition;
- the ability to choose a doctor independently when receiving specialized medical care in a planned manner in an inpatient setting;
- stay in a ward with an increased level of comfort and service at the patient's request.
Primary and emergency medical care does not require any referrals. You can visit a number of specialists without a referral, including:
- obstetrician-gynecologist;
- psychiatrist;
- a narcologist;
- dentist;
- pediatrician;
- a phthisiologist;
- a physician who has a patient with a chronic disease under his/her care.
The procedure for providing medical services to foreigners or stateless persons is also changing. In particular, "the cost of medical care provided to a foreigner or stateless person temporarily staying on the territory of Ukraine is determined by the health care institution that provided it independently."
"Payment for the cost of medical care provided to foreigners and stateless persons temporarily staying on the territory of Ukraine is made in non-cash form in the national currency," the document says.
