According to Kotenko, in order to fill out an online questionnaire about a missing person, you need to open the official website of the Ministry of Internal Affairs of Ukraine, select the "Activities" tab and follow the link to the section of the Secretariat of the Commissioner for Persons Missing in Special Circumstances.
Next, you need to select the active link to the section "Questionnaire of a person who went missing under special circumstances", where the applicant will be offered to log in to the Single Window for Citizens using an electronic signature, Diia.Signature or Bank ID.
He said that after authorization, to file an application for a missing person, you need to use the active button "Order a service" and in the next window you should select the service "Submission of a questionnaire about a person missing under special circumstances to the Commissioner for Persons Missing under Special Circumstances".
Next, the applicant should fill out a questionnaire about the civilian or military person with whom they lost contact due to the war.

Since May last year, data on missing persons has been entered into the Unified Register of Missing Persons. The register was developed by the Ministry of Internal Affairs in cooperation with the Commissioner for Missing Persons and other agencies.
Since the beginning of the Russian invasion, the number of disappeared persons has increased 4 times and today more than 50 thousand people are considered missing.
